Report an incident
Something has happened: someone hurt, something damaged, or a near miss where nobody was hurt this time. Report it with enough detail that someone can act on it.

Incident, near miss or hazard?
- Incident: it has already happened. Someone was hurt, or something was damaged.
- Near miss: it happened, but nobody was hurt this time. Report it the same way: it is the warning before the next one. Pick Near miss as the type.
- Hazard: it has not happened, but it could. Use Report a hazard.
Step by step
- Tap Report at the bottom of the screen, then Report an incident.
- Pick the type (Near miss, First aid, Medical treatment, Property damage, Environmental, Lost time) and how serious it was.
- Say exactly when and where it happened, so someone can find the spot.
- Add everyone involved.
- Describe what happened, in detail. Then what was done straight away.
- Add photos, and tap Submit Incident Report. It goes to your foreman, who reviews it and passes it up to management. Anything critical goes straight to management.
What a good report says
Write it for someone who was not there. A good report answers:
- What happened, step by step, and what was damaged or who was hurt. Say so if nobody was hurt.
- How it happened, and anything that played a part: a spotter missing, poor visibility, a rushed job, the weather.
- Exactly when and where, down to the spot on site.
- What was done straight away: first aid, equipment stopped and locked out, the area made safe, and who was told.
The example in the video:
What happened: At about 9:40 the wheel loader was reversing out of the laydown yard toward the east gate. Its rear counterweight hit the guardrail beside the loader route. The rail is bent at the impact point and the post nearest the hit is loose at its base plate. The counterweight is scraped. The spotter had stepped away to wave in a delivery truck, and the reversing camera was fogged. Nobody was hurt.
Immediate action: Loader parked and locked out. Area coned off and the rail taped. Foreman told at 9:45. The loader stays parked until the mechanic checks it, and the route stays closed until the rail is replaced.
