Report a hazard
Seen something that could hurt someone? Report it from your phone before it becomes an incident. It takes about a minute.

Step by step
- Tap Report at the bottom of the screen, then Report a hazard.
- Give it a short title. Then say what you saw, exactly where, why it is a risk, and anything you have already done about it.
- Pick a category, the site, and how serious it is.
- Add a photo if you can. It helps whoever fixes it.
- Tap Submit Hazard Report. You will see who it went to, and you can follow it under Your hazards.
The example in the video:
Cones down beside the open trench. North approach, by the east end of the storm drain trench. Two cones are knocked over, leaving a 5 m gap beside the open trench, and trucks are passing about a metre from the edge. I stood them back up for now, but trucks keep clipping them. It needs a solid barrier.
Good to know
- Hazard or incident? A hazard could still hurt someone. An incident has already happened, and so has a near miss: report those with Report an incident.
- Who gets it? Low and Medium go to your foreman. High and Critical go to your foreman and the safety lead right away.
- Critical means someone could be seriously hurt right now. If anyone is in danger, tell your foreman in person as well.
